1. The Choice Method Conception Chart is provided by Choice Baby Concept as master licensee and distributor subject to these terms and conditions and on the understanding that the applicant has provided correct information on this Application Form and paid the costs for the conception chart.
2. Key questions on the Application Form are marked with an asterix and any incorrect or false answer to these questions will incur payment for another conception chart. It is the responsiblity of the applicant to ensure that her personal information (name, date of birth and blood group) and answers to or the key questions in the Application Form are correct.
3. The Choice Method Conception Chart is strictly for pre-conception gender selection. It is not a medical tool medical advice for avoidance of sex linked diseases or conditions or illness.
4. The Choice Method Conception Chart only show the favourable days to attempt conception naturally for gender selection. It has no link of any kind to factors producing spontanenous abortion, miscarrieages, pregnancy loss, premature birth, birth defects, congenital abnormalies, chromosomal aberrations in the foetus or in the neonate, or any other abnormality and the company and its licencees do not accept any responsiblity whatever for these conditions and for conceptions and for pregnancies with such outcomes.
5. The Choice Method Conception Chart is not a fertility chart as the polarity cycle on which it is based is independent and seperate from the ovulation/menstrual cycle.
6. The company provide a 100% money back guarantee to any mother who deliver the opposite gender to the on requested for in this form. The applicant is required to submit a certified true copy of the child's Birth Certificate certified by either a Commissioner for Oaths or Lawyer within 90 days of birth of the child to the company.
7. The company shall refund the full amount received from the applicant for the conception chart within 60 days from receipt of the certified true copy of the child's birth certificate showing the opposite gender to the one applied for and shown on the Choice Method Conception Chart. The fee for the Choice Method Conception Cart is non refundable except in the case of the full refund as hearin stated.
8. The company and the applicant agree that in the event the conception results in an opposite gender from the one applied for, the only indemnity and liability is the payment of full refund as stated in item [7], and the applicant shall indemnify the Company and its Licencees frm any legal action by third parties.
9. A valid contract comes into existance between the Applicant and the Company upon receipt of payment and accceptance of this Application Form by the company.
10. The parties hereby agree to adhere to all terms and conditions of this Agreement.
